Full payment is due at time of service.
We accept several forms of payment for dental treatment provided:
- Cash, Check, Money Order, Visa, Master Card, and Discover.
- Payment plans are available based upon approval.
- There will be a $35.00 fee on all returned checks.
Do You Have Insurance?
As a courtesy to you we will help you process all your insurance claims. Please understand that we will provide an insurance estimate to you, however it is not a guarantee that your insurance will pay exactly as estimated. Your insurance company and your plan benefits ultimately determine the amount paid. We will, of course, do all we can to make sure your estimate is as accurate as possible.
Be aware that your insurance company does not guarantee payment over the phone. We will not know the exact amount they will pay until they respond to the claim.
All charges you incur are your responsibility regardless of your insurance coverage. We must emphasize that as your dental care provider, our relationship is with you, our patient, not with your insurance company. Your insurance policy is a contract between you, your employer, and your insurance company.
Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area. You are responsible for payment regardless of any insurance company’s arbitrary determination of usual and customary rates.
There are a few carriers that will not send payment to the provider even if we request it.
If insurance fails to pay benefits as anticipated, the remaining balance is required to be paid within 30 days of the final billing date. At our discretion, any unpaid balance after 90 days will be sent to a collections agency at which the patient is responsible for any fees associated with the collections of the balance. Our office will not enter into a dispute with your insurance company over any claim. We will cooperate fully with the regulations and requests of your insurance company that may assist with the claim being paid.
Cancellation/No Show/Late Arrival Policies
Reserved appointment time in any dental office is limited and valuable. We will work to confirm your scheduled appointment time using a variety of methods including online confirmations, text and calling. It is extremely important that all patients honor their reserved appointment time. Failure to provide us with a 2 business day cancellation call will result in a $50 fee being charged. Without adequate notice we do not have the opportunity to reschedule that time with another patient who has a dental need.
No Show Policy:
If you do not call or show up for your scheduled appointment a $50 fee will be charged to your account. Missed appointments will be documented in your dental record. If you miss more than three dental appointments you will no longer be able to schedule an appointment for routine care. You will be able to be seen as a walk in, but appointed patients and emergencies will take priority.
In an effort to serve our patients in a timely manner, we ask that you are on time for your scheduled appointment. In the event you are running late, please call the office. If you are more than 15 minutes late to your scheduled appointment, you may be given one of the following options:
- If possible, have a portion of the scheduled dental work completed during the remaining time.
- Wait for an opening in the schedule that will allow the work to be completed.
- Reschedule the appointment.
